Warren County’s Community Health Needs Assessment

The State of New Jersey requires all counties to conduct a Community Health Needs Assessment (CHNA) and develop a corresponding Community Health Improvement Plan (CHIP).

This edition of the Warren County CHIP was developed in partnership with Strategic Health Solutions, LLC and the Warren County Community Health Improvement Coalition (WCCHIC), a group of government agencies, local non-profit partners, and additional community stakeholders.

The strategies identified in the plan will help WCHD and its WCCHIC partners address the major health challenges facing the residents of the county. WCHD looks forward to working on a variety of initiatives and supporting local partners to promote health and wellbeing for all Warren County residents.
