• Must have attended required virtual Technical Workshop for Final Report scheduled on Wed., Oct. 22, 2025. (Links to be sent to re-grantees prior to the meeting.)

Reports due no later then 3pm on Fri., 12/19/2025, unless your organization received an extension for December programming. If so, please submit following programming, before 12/31/2025.

• Final Report packet (PDF): LAP-FR 25
• Financial forms (EXCEL): LAP-FFR 25
• Supplementary Materials (receipts/invoices, copies of print material, screenshots of websites/Social Media, copy of marketing & advertising, up to 4 color photographs, etc.
